A highly reputable home health and hospice organization in New Hampshire is searching for a Registered Nurse Case Manager for its Littleton/Whitefield territory. In this role, you will provide direct patient care, complete initial and ongoing assessments, plan & document care, and communicate with other healthcare providers and community resources as needed. Responsibilities:
- Deliver quality care to high-acuity patients, including IV infusions, wound vacs, and pain control.
- Work one-on-one with patients outside of a clinical setting.
- Design, implement, monitor, and evaluate patients’ care plans.
- Coordinate services and inform MDs and necessary healthcare professionals of changes in patient’s conditions.
- Provide ongoing education and support to patients and families.
- Administer medications as needed or directed.
Qualifications:
- Graduate from an accredited nursing program.
- A BSN is preferred.
- Active RN License
- 2 years of home health experience is preferred.
Additional info:
- Full-time, permanent position
- Schedule: Monday-Friday, days
- Compensation: $33-52/hr., depending on experience, incentives!
